This team will carefully evaluate the medical condition of the patient, their current home situation and their future needs from a home care provider. The existing medical conditions, treatment goals and treatment history will all be used during the assessment. The information that is gathered will be combined with the patient’s history from the primary care physician to create a plan of care for the best clinical outcome.
